Senior Health Review
Overview
The Senior Health Review is a quarterly document used to record any changes in client health status or care needs.
Applicable Staff
ID Staff nurses may complete, edit, and view the form. All ID staff may view the form.
Usage
The Senior Health Review Form is client and episode based and may be accessed from the Home View, within a client's chart, or via the menu path Avatar CWS/Mercy Modeled Clinical Forms/Mercy ID Forms/ Senior Health Review. Select the appropriate client if opening from outside of a specific chart. The form opens to the Quarterly Senior Health Review Section and the Health Status subsection.
Health Status Subsection
Enter information about any significant health changes, current medications and procedures, dietary needs, and self-medication status.
Personal Care Needs Subsection
Record information regarding hygiene, toilet care, eating skills, mobility, respiratory aids, and any other needs required.
Recommendations Subsection
Record recommendations for Medical Follow Up and future staff training. Press the blue Submit button when complete.

Any useful tips, and questions provided by users after roll out.
